Terms & Disclaimers
1. Physical Therapy Services
All physical therapy services are provided by a licensed physical therapist and are intended for the evaluation and treatment of injury, pain, or physical limitations. These services are considered skilled, medically necessary care.
- Team Rally does not bill insurance or submit claims on behalf of clients.
- We do not participate in any insurance networks.
- We do not accept Medicare or Medicaid. Individuals with these benefits are not eligible to receive physical therapy services from Team Rally under federal guidelines.
By scheduling a physical therapy session, you acknowledge that these services are provided on a cash-pay basis.
2. Performance & Wellness Services
Performance and wellness services are non-clinical in nature and do not include evaluation or treatment of medical conditions, injury, or pain. These services are intended for athletes and active individuals who are medically cleared for full physical activity.
Examples include:
- Return-to-sport coaching
- Strength & conditioning programs
- Force plate performance testing
- Team/club screenings and workshops
These services are not physical therapy, are not covered by insurance, and are not eligible for Medicare or Medicaid reimbursement. Participation does not establish a patient-provider relationship.
3. Medical Clearance
Clients engaging in performance and wellness services must be medically cleared to participate in high-intensity training and testing. If you are experiencing pain, injury, or health concerns, please seek medical or physical therapy services first.
4. Cancellation & No-Show Policy
We kindly ask for 24 hours’ notice for all cancellations. Appointments canceled with less than 24 hours’ notice, or no-shows, may be subject to the full session fee. Exceptions are made for illnesses, emergencies or unexpected transportation issues.
5. Privacy
All personal and health information shared with Team Rally is handled in accordance with HIPAA regulations and industry best practices. We do not share your data without your consent.
